18-25 year old female with a right complete cleft lip and palate who wanted to improve the asymmetries of the nose, lip, and flatness of the midface. The patient underwent a cleft revision rhinoplasty with rib cartilage graft and fat transfer to the lips and midface. In the post-operative frontal views, you can see a straighter nose, a lifted tip, and the right nostril has been brought down and inwards. In the profile views, you can see the dorsal hump was lowered, and the tip of the nose has been lifted, rotated, and refined. The fat was placed in the cheeks and upper lip to improve fullness since midface deficiency is common in cleft patients.
